Internal Applicants Only The Social Work Service assess the needs of vulnerable adults and arrange the necessary supports to enable people to remain living in their own homes, wherever possible, with independence and dignity. We work closely with the individuals we support and their families/carers to ensure that what we do to help reflects their own aspirations whilst receiving the help they require based on their assessed need. We monitor the supports provided and ensure they continue to meet the needs of the individual service user. You will undertake assessments of need and risk as directed by the Line-Manager/registered Social Worker and where the service deems appropriate. You will also carry out the functions associated with care planning and review. Experience of working with vulnerable people is essential and you must have 3 Standard Grades or equivalent including English.
