Register a still-birth

A still-birth can be registered in any Registrar’s office in Scotland within 21 days.

Please use the contact details below to make an appointment to register the still-birth at one of our registration offices.

If the parents of the baby are married to each other, either can register the still-birth.

If the parents of the baby are not married to each other, and the father's details are to be recorded on the still-birth certificate, both parents have to sign the still-birth register. If the father is unable to attend to register the still-birth please ask a Registrar for advice.

You will need to provide the following documents when you come to register the still-birth:

  • Form 6 issued by the hospital or Midwife
  • Birth certificate(s) of parent(s)
  • Marriage certificate if the baby's parents are married

If some of these documents aren't available - speak to the Registrar, who will be able to advise you.


A full extract of the still-birth is issued free at the time of registration and for one month from the date of registration. The cost of £15 will be charged thereafter.
